All new Maclay students must present a certification of a school-entry health examination (State of Florida School Entry Health Exam Form DH 3040) performed within 1 year before enrollment in school, and provided to the school within 30 days of entry (F.S. 1003.22).

Out of state health examination certifications may be accepted only if they meet the same assessment requirements as the State of Florida School Entry Health Exam Form DH 3040 and have the physician's signature and office stamp.

For all new students, rising Kindergarten students, and rising 7th grade students, proof of immunizations (or medical exemption) must be documented and submitted on the Florida Certification of Immunization (form DH 680) (F.S. 1003.22). This form is available through your child’s physician or may be downloaded from the Florida Shots website.

A Certificate of Religious Exemption form DH 681 may be presented to the school (in lieu of form DH 680) if the student is seeking exemption on religious grounds (F.S. 1003.22).

All required documents must be on file for your child before the first day of school.

Additional Immunization Requirements for 7^th Grade

All the immunizations listed above plus 1 Tdap (Tetanus, Diphtheria, Pertussis) booster vaccine.

Student medications will be stored in the clinic and dispensed under the supervision of the school nurse. Students are not allowed to self-administer or carry medications on campus or for school functions (field trips, athletic events, etc.). Students may carry and self-administer rescue medications (EpiPen and inhalers) only if it was previously discussed with and approved by the school nurse.

Seizure Action Plan - If your student has a history of seizures, the Seizure Action Plan (containing multiple required forms) must be filled out by the prescribing physician and signed by the physician and parent/guardian at the start of each new school year. 

EpiPen Action Plan - If your student has a history of severe allergies and requires an EpiPen, the EpiPen Action Plan (containing multiple required forms)  must be filled out by the prescribing physician and signed by the physician and parent/guardian at the start of each new school year. 

Other Medications - The Permission Form for Administration of Medications at School is to be filled out by a parent or guardian and dropped off with the medication at the start of each new school year. If your child is prescribed an inhaler that you prefer they self-carry and self-administer, the Authorization to Carry and Self-Administer Form must be completed by the prescribing physician and signed by the physician and parent/guardian at the start of each new school year. 

All prescription medications (including EpiPens) will need to be in the original packaging/bottle with a pharmacy label detailing the student's name, medication name, and dosing instructions; over-the-counter medications will need to be in the original packaging with the dosing instructions.

Any prescription medications missing a pharmacy label will not be accepted.  Please check expiration dates prior to dropping off medications - expired medications will not be accepted.

The electronic Vital Health Record is required for every student before the first day of each new school year. To complete these online back-to-school forms, please log into your Veracross Parent Portal at https://portals.veracross.com/maclay/parent, click Magnus Health on the right-hand side, and then click "Complete now".

The goal of Maclay's Student Health Center is to promote successful learning conditions by encouraging a healthy environment for all students, faculty, and staff. Please do not send your child to school if they are sick. Please see our Sick Day Policy below - reference these guidelines when determining when to keep your student home from school/school-related function (field trip, athletic event, etc.).  

Students, faculty, and staff should stay at home if they are experiencing any of the following:

• *Fever (100F or higher)
• Acute cough
• Acute congestion or runny nose
• Sore throat
• *Nausea or vomiting
• *Diarrhea
• Fatigue
• Body aches or chills
• Shortness of breath or difficulty breathing
• **Rash
• New loss of taste or smell

*Students experiencing fever may return to school once symptoms are improving and fever-free without medication (Tylenol/acetaminophen or Motrin/Advil/ibuprofen) for at least 24 hours.

*Students experiencing vomiting and/or diarrhea may return to school once symptoms are improving and vomiting/diarrhea has been resolved for at least 24 hours.

**Students with a new rash (especially honey-colored crusting, rashes around the hands/feet/mouth, open lesion or oozing, hives, or raised bumps) must be evaluated by a physician prior to returning to school - a doctor's note is required upon return.

Guidelines for Return to School for Common Illnesses

Flu – students must stay at home until symptoms are improving and fever-free without medications for at least 24 hours. Generally, students diagnosed with influenza will need to stay at home for at least 5 days from the onset of symptoms.

Strep Throat - students must stay home for at least 24 hours after the start of antibiotics and/or cleared by a physician to return to school.

Impetigo - students must stay home for at least 24 hours after the start of antibiotics and/or cleared by a physician to return to school.

Conjunctivitis ("Pink Eye") - students must stay home for at least 24 hours after the start of antibiotics and/or cleared by a physician to return to school.

Molluscum - students must stay at home until evaluated and cleared by a physician - all exposed bumps that are not easily covered with clothing must be covered with a bandage or medical tape while attending school or any school function.

Hand, Foot, Mouth Disease (HFMD) - students may return to school once symptoms are improving, fever-free for at least 24 hours without medication, and sores/blisters dry up.

Hives - if a student experiences a sudden onset of hives while at school with or without a known trigger and Benadryl is given by the clinic (only after getting permission from the parent or guardian), then the student must go home immediately due to the sedative effects of Benadryl. Upper School students and faculty/staff are not permitted to drive themselves once Benadryl is administered due to risk of injury.

Lice - Maclay School has a no nit policy. Students may return to school after treatment and after being cleared by the clinic staff. The clinic must check the student's scalp PRIOR to returning to the classroom - students must go home for any presence of live lice or nits.

• Frequent hand washing with warm water and soap for at least 20 seconds. Use hand sanitizer only if hands are not visibly soiled and soap/water is not readily available.
• Disinfect surfaces and items that are frequently used or touched.
• Cover coughs or sneezes with the bend of your elbow or tissue.
• Avoid touching your eyes, nose, and mouth.
• Please stay home if you feel sick.
