TO:      All Student Athletes and Parents of New Students and Transfer Students

RE:      Athletic Eligibility and Requirements for Athletic Participation



Only students who are enrolled for the 2017-18 school year at Maclay School will be eligible to participate in summer workouts. This includes team workouts, team camps, open gyms, and use of the Maclay weight room.



In order to represent Maclay School on any sanctioned athletic team or club, the following must be completed:


1.) The Athletic Director must have on file the following:

a. FHSAA (EL2) Pre-participating Physical Evaluation Form (good for 365 days)

b. FHSAA (EL3) Consent and Release Form  

c. Maclay School Emergency Treatment Authorization Card (must be notarized)


These forms may be found on the Maclay School Website under Athletics


2.) Maclay School must have a copy of a birth certificate from the State of birth. A student may participate in athletics at the high school level until the day he or she reaches the age of 19 years 9 months.


3.) The student athlete is limited to four consecutive years of eligibility from the day he or she begins ninth grade.


4.) The student must have and maintain a 2.0 Grade Point Average based on the standard grading scale as defined by the Florida Department of Education.



In order to represent Maclay School or any sanctioned athletic team or club, the following must be completed:

1.) The student must be enrolled and in attendance on or before the first day of practice for whichever sport he or she is planning on participating in.

2.) The following forms must be completed and notarized. These are in addition to the ones listed above.

a. FHSAA (EL6) Notice of Transfer

b. FHSAA (GA4) Affidavit on Recruiting
