The Student Health & Emergency Record Update form is available on-line this year in order for you to provide us with your most current emergency contact information, pertinent health information about your child and your signature giving us your permission, if you choose, to administer certain over the counter medications as listed on the update form.
You will be able to access this on-line form beginning August 9 by following these instructions:
New Families: You must create your parent portal log-in first. You will receive an email Tuesday morning with your log-in instructions.
- Log-in to your parent portal account at https://school.maclay.org or click the grade book icon on the Maclay homepage.
- Click the Forms link on the left-hand side
- Click on Confidential Student Health Update file
- Update any information
- Type in your signature and submit
A new record update form must be completed and submitted for each child in your family annually.
This is a valuable tool that assists us in being able to provide your child with prompt and efficient attention when needed during the school day. The on-line form will need to be submitted before the first day of school.
